L2 Misdirected Pedicle Screw

 

This is an older gentleman who underwent a long fusion for degenerative scoliosis.  The right sided pedicle screw at the L2 level is clearly well into the spinal canal. Although the L2 nerve root is generally quite sensiitve, he remained entirely assymptomatic from the misdirected screw.  Due to irritation from the hardware in general, the instrumentation was removed over one year later.  With removal of the misdirected pedicle screw, a rush of CSF was encountered indicating the screw was intradural; again, without noticeable radicular signs or symptoms.
